3rd Trimester
Gestational Age: 28 weeks until Term
3rd Trimester Fetal Growth
Indications
- Small/Large for Dates
- IUGR or History of IUGR
- History of Macrosomic Infant
- AMA
- Maternal Diabetes
- Abnormal prior fetal ultrasound and/or screening labwork
- Maternal Obesity
Protocol
During the 3rd Trimester Fetal Growth
- Fetal Biometry to obtain EFW: HC, BPD, AC, FL
- Fetal Presentation
- Amniotic Fluid Index (AFI): Deepest vertical pocket of amniotic fluid in each quadrant
- Placenta
- FHR
- Stomach, Kidneys, Bladder, 4CH, LVOT, RVOT
- Some additional fetal anatomy may be required depending on individual facility protocols

Fetal Breathing
Biophysical Profile (BPP)
3rd Trimester
Gestational Age of 28 weeks and Greater
Can be done sooner than 3rd Trimester if Indicated
Biophysical Profile (BPP)
Indications
- Maternal Diabetes
- Multiple Gestation
- IUGR
- Fetal Abnormality/Aneupoloidy
- History of Intrauterine Fetal Demise(IUFD)
- Low AFI
- Decreased Fetal Movement
Protocol
During the Biophysical Profile
- Fetal Presentation
- Placenta
- FHR
- BPP Score Total = 8 Points, 2 points for each of the following
- Fetal Breathing: Diaphragm moving up and down continuous for 30 seconds
- Fetal Tone: One episode of extension and flexion of limbs or open/closing of hands
- Gross Fetal Movement: Three gross body movements
- Amniotic Fluid Index (AFI): Deepest vertical pocket of amniotic fluid in each quadrant
- A failing score is typically considered 4 out of 8 points - The provider may request the exam to be extended another 15 minutes, or the patient may be sent for an Non-Stress Test (NST).
